As a business, you will need to provide an ID that contains your business name and address.

As an individual, you will need to provide two forms of ID for yourself - one must be a photo ID and one must have your home address. Acceptable Forms of Identification are: Driver’s License, State ID, Armed Forces ID, Government ID, University ID, Recognized Corporate ID, Passport, Alien Registration Card, Current Lease, Mortgage or Deed, Voter Registration Card, Utility Bill, Home or Vehicle Insurance Policy, Vehicle Registration.

Please note that Credit Cards, Social Security Cards, and Birth Certificates are all expressly prohibited as ID.
